The importance of the setting, Smitty's hospital room, is the events that have taken place in the room, and the changes that each character has experienced while being there. After Smitty and Ginny share a kiss at a party, something inside of him breaks out. This leads to Caulder and Ginny figuring out the biggest mystery about Smitty's life. Getting psychiatric help, Smitty finally tells his story to the doctor, which is passed onto Caulder and Ginny knowing about the situation. They are told to visit Smitty everyday to help him adapt and to learn more about his story. What they end up learning is much more than what they thought. Ginny ends up learning Smitty's real name, which is Michael, and Smitty and Ginny also realize that they love each other and that they want to look after each other in the future.
