This paper will also discuss what internal and external factors there are that may affect the organization, the importance of servant leadership in an organization, and organizational structure.
Family HealthCare Network Family HealthCare Network was established in 1976 and is a private, non-profit community based organization. This organization has federally qualified health center sites located in Ivanhoe, Visalia, Porterville, Woodlake, Springville, Three Rivers, Goshen, Hanford, Cutler-Orosi, Farmersville and Terra Bella. Some of the services that Family HealthCare Network provide is Family Medicine, Internal Medicine, Pediatrics, Obstetrics, Gynecology, Family Planning, Dentistry, and Nutrition. The Family Health Center is a clinic that is under the umbrella of Bear Valley Community Hospital, a small rural hospital. The hospital is located in a ski resort town in the mountains above San Bernardino County. The next closest medical facility is an approximate hour drive on curvy mountain roads. The hospital is a standalone hospital, not associated with any other hospitals or chains. Bear Valley Community Hospital provides emergency room services, has eleven acute beds and a skilled nursing facility. Bear Valley Community Hospital accepts a wide variety of insurances and sees all ages and types of patients. The Family Health Center has a variety of specialty providers, as well as family practice. They have doctors with specialties in internal medicine, family practice, obstetrics/gynecology, pain control and chiropractic services (Family Health Center, n.d.). There is a telemedicine program that currently provides psychology services through telemedicine (Family Health Center, n.d.). There are several Family Nurse Practitioners and a Pediatric Nurse Practitioner (Family Health Center, n.d.). As a result they see patients from pregnancy through adulthood. A physician named Avedis Donabedian (2005) proposed a model for assessing health care quality based on structures, processes, and outcomes. He defined structure as the environment in which health care is provided. This is known as the organizational characteristics such as the measurement of staffing ratios and the number of hospital beds. The process is described as the method by which health care is provided. This represents the communication and interaction seen between doctor and patient. The Community Health Center provides a service called Better Health Clinic that can enhance adult health and wellness by providing contraceptive services, pregnancy testing and counseling, achieving pregnancy, basic infertility services, preconception health, sexually transmitted disease services, breast cancer screening, cervical cancer screening, and other preventative health services. Social services can assist with applying for W.I.C. and ACA. They can also render assistance with adoption and stay at home programs.
Improving the quality of health care system is the main goal of this organization. In this case study we will be talking about the strategic plans being made by the organization for the next decade to deal with the problems of resource management, network growth, patient satisfaction as well as nurse staffing. The readiness of the organization towards catering the citizens' needs for health care will also be discussed in this case study (Goetsch and Davis, 2010).
The superior act of living an effective life lies in the pursuit of excellent vision, to move from where we are to where we ought to be, either as a person or as an organization. A health care leader must motivate the organizational team, find how to work with the associates to effectively dedicate their time and energy to achieve the identified goals and objectives through the established vision. The team members need to buy into the vision, where they are currently doing what the leader envisions, the leader must encourage them, keep them on track and set standards for the team by setting goals and objective, flowing from the vision. This will enable the leader to move the health care organization to the expected performance level within the set time period. The leader must ensure that individual responsibilities of the associates are identified, motivated and resources provided to achieve the identified goals. To pursue the vision, I will lead the development of the health care organization’s strategic alignment of clear goals and objectives, to preserve the core mission and encourage progress toward the envisioned future state. Also, I will encourage and provide the direction for the organization without hindering the development of the goals that we expect to accomplish.

From my understanding UMN Family Medicine Residency Programs has a long history with the goal of making a difference in the community by providing affordable quality health care for everyone in the community regardless of their socioeconomical status. It also emphasizing on integrating primary care and mental health care which is an important and growing concern in every community, though unfortunately it doesn’t get the attention it deserve in most primary care settings. I find this program values and objectives alien with my future career goals. I believe being part of this program will give me to opportunity to gain comprehensive clinical experience while work with in a community and grow better understanding of the needs of divers population.
