2 Patient Home Safety Assessment Patient health outcomes can be influenced by home safety, thus necessitating evaluating the client's living environment to identify potential threats. The patient's functional activity can be assessed by tools such as the Barthel Index, which estimates the Basic Activities of Daily Living (BADLs), and the Lawton Instrumental Activities of Daily Living. According to Pashmdarfard and Azad (2020), Barthel Index evaluates activities such as dressing, bathing, toileting, grooming, and feeding, while Lawton Instrumental Activities of Daily Living (IADLs) entail activities such as administering medication, telephone use, housekeeping, preparing meals and laundry. Indicators such as falls, forgetfulness and memory loss, poor hygiene status, progressive unintentional weight loss, and difficulties adhering to medications indicate additional care. According to Stepko (2021), incapacitation of the patient's mobility, changes in appearance, and depression are the main indicators of the patient's need for additional help or transition to a care environment. The nurse practitioner serves as the patient's advocate and provides significant information to the friends and family concerning the patient's well-being and quality of life. Some of the NP's common roles in care transition include identifying the care needs, educating the family members on the care requirements, identifying barriers to effective care, and referring the patient for checkups and monitoring to improve overall health. The NP can make referrals to assisted living facilities and home health agencies. The NP can provide the patient's treatment records and medications to promote continuity in the treatment process and inform the facility on the care continuum and transition in care.
