As a clinical requirement for my Adult 1: Medical-Surgical course, I had the opportunity to observe a patient in the Operating Room and in the Post Anesthesia Unit of Advocate Good Samaritan Hospital. Best practice guidelines are evidence-based recommendations that assist practitioners with assessment and management of appropriate healthcare (Taylor, 2012). One area that utilizes best practice to ensure excellent patient outcomes is ostomy care. The patient may have an ostomy for a number of reasons. They can be either temporary or permanent and are for patients who have bowel or bladder cancer, inflammatory bowel or diverticular disease, trauma, or perforation of the bowel. For ostomy care, it is important to focus on the areas of pre-operative, post-operative and discharge care (Taylor, 2012).

The Leapfrog Group was formed in 2000 in an effort to bridge the gap between of evidence-based medicine (EBM) and practice (Brooke, Dominici, Makary, & Pronovost, 2009). One EBM practice that Leapfrog has promoted is the use of perioperative beta-blockers during AAA repairs (Brooke, Dominici, Makary, & Pronovost, 2009). A random control trial concluded of the hospitals partaking in the study, 26% reported compliance with routine beta-blocker use after surgery, whereas, 74% reported failure to implement the policy. The topic I chose for this journal review is about a condition called Herlyn-Werner-Wunderlich Syndrome (HWWS) with hematocolpos. Herlyn-Werner-Wunderlich Syndrome represents a type of Müllerian duct and Wolffian structure abnormalities. HWWS, can also be referred to as OHVIRA (Obstructed hemivagina and ipsilateral renal agenesis) syndrome (Karkera, 2015). This condition can represent female genital malformations by: uterus didelphys, unilateral low vaginal obstruction, and ipsilateral renal agenesis with all three of these being secondary to mesonephric duct–induced müllerian anomalies (Karkera, Bendre, Ramchandra, D’souza, 2015). What is hematocolpos? Hematocolpos is a condition in which the vagina fills up with menstrual blood and is usually caused by an imperforate hymen. This journal describes how the use of sonography helped diagnose patients with this congenital disorder.
