On 8/13/15, the department received a report on Mr. Gail Beaird and Mrs. Marilyn Beaird. The report stated the couple is married and living by themselves in their Cordova Home. They both require constant supervision and assistance, which is provided to them by Visiting Angels. Mr. Beaird had serious anger issues and reportedly has a long history of domestic violence.
On 8/13/15, Mr. Beaird threatened to shoot and kill himself, Mrs. Beaird, and anyone else in the house. Mr. Beaird is constantly running off sitters, and has already fired10.
The worker met with Mrs. Marilyn Beaird on 8/18/15. She is bed and wheelchair bound and requires two assists when transported. Mrs. Beaird has diagnoses of Vascular Dementia and is unable to answer questions

Gail Beaird on 8/19/15. When questioned about the situation that surrounded him ending up in Senior Care he stated, "I don't remember." Mr. Beaird denied ever having an episode where he could not remember what happened before, and denied threatening to kill anyone. He stated, "The only time I threatened to kill anyone I was kidding." Mr. Beaird admitted he requires some assistance with ADLs due to his inability to walk complications from diabetic neuropathy. He also admitted to firing one or two Vising Angels because they were sleeping and not doing their job. During interview, Mr. Beaird answered all questions appropriately and independently.
Medical History includes UTI, diabetes type 2, hypertension, GERD, glaucoma, diastolic heart failure, dysphagia, coronary artery disease with bypass surgery, chronic joint pain, and dementia Alzheimer's type.
May 18th 2014 Mr. Beaird fell and broke his hip. This followed hip surgery performed by Dr. Baker and 21 day rehabilitation form Cordova NH. Before his 21 days was up Mr. Beaird tried to break out of NH by running his scooter though the front door at the NH. When the scooter hit the door, the door opened and Mr. Beaird rolled onto the front porch during this altercation, his foot was injured and the ambulance was called. Joey Beaird and Mrs. Julie Johnson moved Mr. Beaird home and set up for Visiting Angels to assist with his care at home. Mrs. Beaird joined Mr. Beaird at home in July of 2015. It was also during this time Mr. Beaird began to refuse medication for anxiety. Then on August 13, the department received the report stating Mr. Beaird threatened to kill his daughter, wife, himself, and anyone else in the home.
The department has talked with Mr. Joey Beaird, Mrs. Julie Johnson, and Visiting Angles. The children are not willing to move into the family home 24/7 and assist with Mr. and Mrs. Beaird's care. Ms. Johnson is afraid of her father and does not believe it would be a safe environment for herself or her mother and Visiting Angels have stated if Mr. Beaird returns home they will be required to stop services due to fear of Mr. Beaird. currently no one is willing to care for Mr. Beaird at his residence and Mr. Beaird requires assistance with activities of daily living and 24/7 care.
It should be noted that during almost all violent outburst Mr. Beaird has been diagnosed with a UTI. He had the catheter removed and now wears a urine bag on his leg connected directly to his bladder. Removing the catheter has helped with number of UTI Mr. Beaird experiences. However, the bag must be emptied and cleaned multiply times throughout the day and Mr. Beaird is not physically able to assist in this daily
A report was received on 7/9/2017, indicating that Tammy Albury (mother) and Armando Guerrero (father) were involved in a domestic dispute in the presence of Cember Albury (CV-15), Devon Albury (CV-16), and Damian Guerrero (CV-1). According to the report, Armando went to Tammy’s truck to retrieve his property. It was said that Tammy went to Armando’s car to retrieve a phone cord and Armando grabbed her by the hair and pulled her by her left arm out of the car. The report indicated that Tammy fell on the ground and Armando got into his car and drove off. The report indicated that Tammy sustain a bruise on her left upper arm around the bicep. According to the report Cember (CV-15), witness Armando pulled Tammy out of the car.
Mrs. Keller, 86 years old was diagnosed with dementia with signs of behavioral problems. She can still walk independently but needs minimum assistance in some basic activities of daily living.
Mitchell went over with them the process of court ordering Mr. Beaird to the nursing home. Mr. Beaird did not agree or disagree with this determination. Therefore, worker asked Mr. Beaird what was his decision about placing Mr. Beaird into a nursing home. Mr. Beaird stated, "It's not something I want to do but I guess it's what needs to happen." Mr. Mitchell also recommended Mr. Beaird and Mrs. Johnson get together and discuss who would be appointed guardian and conservator. Mrs. Johnson stated she would rather be conservator because her and Mr. Beaird do not get alone. Mr. Beaird did not agree or disagree with this statement. During most of meeting, Mr. Beaird would only talk if directly asked a question. When he did talk, he was usually defending his father's actions. Barbara Dawson was forced to leave her hospital room by the staff of the hospital in Florida Panhandle town of Blountstown. She had been admitted for abdominal pain. Dawson responded by saying that she was still feeling very sick, and wasn’t ready to go home. After she had gotten arrested and taken outside into the parking lot, she collapsed and was taken back inside within 20 mins. When a physician went up to Barbara’s car to check on her, she stated that she did not look the same as when she discharged her from the hospital. I called acquiring about assisted living for my grandmother the late Martha Pickens. Martha Pickens is 72, she has lived independently for sense her husband passed away over 15 years ago. I've notice that she needs help with remembering to take her medication and keeping up with house cleaning. I work a new position and live an hour away from her and want her closer to me and need assistance with caring for her ongoing with my new position I have to be much more depending. This is a new admission transferred from Florida got here last night with a history of vascular dementia with the addition of head trauma in 05/2016. The patient has minimal communication skills and all of this is related by her daughter, Linda Abrams. Linda tells me that her mother was born in Brooklyn, graduated from high school, worked both at Macy's and a ladies dress factory that her father owned. Around the age of 22, she married had three children. Linda Abrams lives in the Briar Creek area of Durham. Arlene Lambert the second daughter lives in Reno, Nevada. The third daughter is deceased. The agency became involved with this family due to Willmundrikk Johnson father making allegations stating Willmundrikk's stepfather attacks him, and threatens at home, and the mother doesn't handle the situation. The father also stated Machelle and her husband are cousins. Machelle the mother stated the allegations were not true.
