Even if a person with dementia can no longer make complex decisions, it is likely that they can make more basic decisions. Every effort should be made to get their input wherever possible. Once the person with dementia begins to lose capacity it is important that their carer and/or health care professionals are consulted and involved in decision making. This method is called the multidisciplinary approach. Macmillan Cancer Support (2012) describe a multidisciplinary team (MDT) as a group of doctors and other health professionals with expertise in a specific cancer, who together discuss and manage an individual patient’s care, planning the best treatment for the individual patient. Rosa’s condition has not only affected her life physically but also psychologically. Over time she has become depressed as she feels ‘stuck in hospital all day and a burden on family and staff’. Psychological theories provide evidence-based explanations for why people think, behave, and feel the way they do. Ralph has chosen this song as he loves it. It's a song he'd love to be able to play on guitar. Ralph has stated he loves to play his guitar. However he gets frustrated when he try's to play it on his own. He often has his guitar out when he is relaxing in the unit and has stated he wants to write a song about his mum. Ralph actually sung part of a song he had thought up. The lyrics were very warm towards his mum. However this song was very short and maybe if Ralph gets better at the guitar with these lessons he will be able to put a whole song together about his mum. This would be a very positive event as
I will now talk about each patient needs as they all differ from each other. Nusrat Patel is 19 years old and has learning disability. This means Nusrat has difficulties in keeping knowledge and skills to the expected level of those the same age as her. Nusrat also has epilepsy which is neurological brain disorder when someone has epilepsy, it means they tend to have epileptic seizures, a seizure is a sudden attack of illness. Nusrat has left residential school to receive full time carer from her mum who has stopped working to care for Nusrat. At times this can be stressful so Nusrat attends the community centre on Tuesday and Thursday which allows Nusrat mother to have a break. Maria montanelli is 34 years primary school teacher who is much like Nusrat mother and takes care of her 96 years old mother who has dementia. Dementia is memory loss and difficulties with cognitive development. Being a primary care for her mother Maria feels she not performing at her best ability because of her lack of sleep which occurs when she assists her mother to the toilet several times. The last patient I would like to mention is Alice Fernandez she is 74 years old who recently lost her husband who had lung cancer. Alice doesn't use her pension the right way as she purchases many drinks as an alcoholic and has increased since her husband passed away. She has been prescribed antidepressant tablet by her G.P but made her lethargic this means she's become slow and sluggish.

In this report I’m going to describe the five stages of the life cycle and the anticipated development processes in which occur throughout each stage. I will also look at a service user whom I have cared for in the past and describe the needs associated with their own development. I will also describe how my knowledge and understanding of TWO contrasting psychological perspectives will inform my practice in the workplace.

At the end of life: Many people can die from dementia; however they can also die with another disease which doesn’t relate to their dementia even though they suffer it. Usually, the dementia will be that far advanced towards the end of life the individual may not even know what is happening or may even be too ‘ill’ to understand. However, it is important as a care worker or any professional that is trained in end of life care (palliative care) to ensure the best quality of life until the end. Each individual should be treated equally, yet individually. These individuals must not be overlooked and still have rights as well as everybody else.
I am currently studying HNC Health Care and as part of my course I have to complete a graded unit, this will entail three stages; Planning, development and evaluation. This will be carried out while on placement within a hospital setting, within the planning stage I have to choose a patient and assist them with a nursing activity. Over the year both positive and negative influences have influenced my learning. The skill I have developed for learning is my aspiration is to become a midwife, this influences my learning in the way that it gives me the hunger to learn new things and improve my knowledge and grades. By improving my knowledge I will be able to go onto the next stage of my development. I believe that if I did not have such a desire to be a midwife, it would affect my learning by creating a mental blockage which would stop my ability to take things in and learn as I would become lazy and not interested from the lack of direction in my life. Honesty is something I value in another person, and I have struggled with service users who haven’t been honest about something. I have tried hard to understand the reasons why someone would not be honest with me. Usually after I have spoken to another member of staff, and heard their input and theories I can begin to see why and move on from the incident.
