By 2001 it was brutally apparent that the U.S. Health Care system was in dire need of a reform in regards to quality and patient safety. Following two separate reports issued by The Institute of Medicine (IOM), To Err is Human (1999) and Crossing the Quality Chasm: A New Health Care System for the 21st Century(2001) the U.S. Congress requested the IOM review quality processes across multiple government funded health care programs. And understandably, “these reports described America’s healthcare system as a tangled, highly fragmented web that often wastes resources by duplicating efforts, leaving unaccountable gaps in coverage, and failing to build on the strengths of all health professionals” (Brown J., p. I – 15, 2013). Thus, the Committee on the Quality of Health Care in America released 6 aims to address key dimensions that require improvement in our health care system. These aims propose that our system needs to strive to be more Safe, Effective, Patient-Centered, Timely, Efficient, and Equitable (STEEEP). All of which were created to help overhaul our current health care system and, more importantly, narrow the quality chasm.

Even though there are many different challenges that will define strategic directions of the healthcare industry, five different challenges will be discussed within this paper. These challenges appear in all States and across the whole medical field from clinics, public hospitals, and private pharmaceutical companies. Today’s healthcare focus is on providing patient-centered, safe and effective care for improved patient outcomes. This focus is not only important for the patient’s health, but has become vital for the financial well-being of healthcare organizations. The United States has consistently ranked among the highest in healthcare costs and the lowest in improving patient outcomes when compared to other industrialized nations (Davis, Stremikis, Squires, & Schoen, 2014). Life expectancy in most of the human population today is at its highest, and many of the diseases that had ravaged human society before had become manageable because of medical breakthroughs that had started a few generations ago. To improve the likelihood of successfully implementing an incident reporting system, as with any (QI) measure, it should be planned out, involving organizational members from top to bottom (Burns, Bradley, Weiner, & Shortell, 2012). As with any new policy or procedure, there can be resistance and barriers. However, with open communication and understanding the need for the measure, this can possibly be eliminated. To me, what might be done differently is to increase the number of primary care physicians. According to the slide 25 Issues in Medical Practices, an imbalance between primary care and specialty care. From 1979 to 1999, the number of primary care physicians increased by only 18%, while the number of specialist increased 118%. In the United States, approximately 40.8% of the physicians are generalists and approximately 59.2% are specialists. In addition, nurses the largest group of healthcare professionals has fallen due to sluggish wages, low job satisfactions, inadequate career mobility, etc.
