On Tuesday July 7, 2015, at approximately 3:01 PM, Kiana Beekman, (MFCU Investigator) (Beekman) received a call on the state office telephone from HILL, Lucy (Service Facilitator of Lucy Hill Services (LHS). During the conversation, Beekman asked HILL to clarify her role and responsibilities as a service facilitator, in addition to the role and responsibilities of HARRIS, LaFrance as the Employer of Records (EOR) for Medicaid Recipient DANIEL, Rose and MCGHEE, Inocencia as DANIEL’s aide. She was also asked to provide any documentation of training on timesheet submission and approvals that she provided HARRIS and MCGHEE under the Department of Medicaid Services (DMAS) Consumer-Directed care aide program.

On 7/11/2015, CM did a visual and had client come to the social service office. CM completed Bi-Weekly ILP Review. In the meeting client appears to be wear out, and tired. She was constantly throbbing her forehead, like if she was having headache. CM inquires what the problem is. Client replies “she doesn’t like the shelter food and sometimes she doesn’t eat” CM advised the client to eat and nourished her body. CM also observed that client is depressed but she continues to refuse medical referral to see a psychiatrist and medical doctor. Client continues to mention her son who is in foster care, and the physical altercation she sustained many months ago here at this shelter. Social Services: On 02/17/2017, Ms. Hair and her son Aaron met with the family assigned Case Manager for the family ILP Document Review. Ms. Hair is expected to meet with her assigned Case Manager bi-weekly. Ms. Hair’s next ILP appointment is on 02/02/2017. Case Manager asked Ms. Hair if she is aware that she is not in-compliance with the terms of her ILP, Ms. Hair stated yes. Case Manager asked Ms. Hair if is aware that failed to attend her ILP meetings since 01/23/2017, Ms. Hair stated that she was aware. Ms. Hair last ILP was conducted on 01/23/2017. Ms. Hair stated that the reason that she has not attended her ILP meetings is due to her work scheduled. Case Manager explained to Ms. Hair that it’s understandable that she is working and she

Social Service Meeting: On 10/25/2016 Ms. Williams met with assigned Case Manager for the family weekly ILP Document Review. Ms. Williams’ next ILP Document Review appointment is on 11/01/2016. Ms. Williams is in-compliance with the terms of her ILP. Case Manager asked Ms. Williams if she has any issues or concerns that she would like to discuss during meeting. Ms. Williams stated no. Children were present at the time of the meeting.

On 8/7/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting; she was alert, satisfactorily groomed, and casually dressed. In the meeting she was cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly ILP Review. Client reported she is exhausted from running around obtaining relevant documents to be submitted to Lemle & Wolfe, Inc. by 8/3/2015. She continues to reports she has Straight Medicaid and she is searching for a Managed Cared that cover oxygen tanks.

I-CM informed the client that he had contacted and left a message with LA Family Housing the week prior, but no one answered due to the Thanksgiving holiday. CM contacted LA Family Housing again with the client present, but on one answered and a voice message was left. CM continued to assess client’s mental health and medication compliance. CM inquired what outside activities the client has been engaging in. CM inquired about client’s plans for Thanksgiving.

Unlike the other case managers Kimberly’s client intel are predominately children and their caregivers, which is a part of the intake process of a new case. In fact, the Kimberly is only assigned to case for approximately 60 days, depending on the completion of the Child Protective Investigator’s report. As a results of children being placed within the Treasure Coast districts from other counties and seldom other states Kimberly is required to conduct monthly visitation as well as documenting the progress or findings in regards to these individual cases. Which is the duties of the Out of County Supervisor.

On 1/5/2016, CM met with the client to complete Bi-Weekly ILP Review and housing plan. In the meeting client was cooperative and friendly. She was dressed appropriately for the weather.

On 11/06/2015 Ms. Tameca Chandler came to the SSO to meet with CM Watterson for an ILP meeting. HS De La Torre approach client to ask her if it is possible to stay extra half an hour in order to develop a Housing Plan and discuss barriers, housing opportunities to transition into permanent housing. Client agreed to stay to discuss a housing plan.

CM was out for the period of 5/22/2015 to 6/9/2015. On 6/11/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting. She was dressed appropriately for the weather. She appears to be friendly and cooperative. In the meeting the client was very talkative. She mentioned to CM her niece gave birth to a baby girl and she can wait to see the baby. She also mentioned to spoke with her sons last night and everything is well with her family. CM inquires if client participate in the onsite Recreation Activities. Client reported she doesn’t participate in the some of the activities. She prefers to go to Bread of Life for food and socialization.

On 7/15/2015, CM conducted a search via Security to locate client. Client was found sitting in the patio. Security asked the client to go upstairs to the 3rd floor to see social services. CM met with the client and completed Bi-Weekly ILP Review. In the meeting the client was well groomed and dressed appropriately for the weather. She appears to be calm, cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly IlP. Client replies she is doing well and she is still waiting for DHS Manifest if she was approved or disapproved for the apartment” CM reiterates to the client as per P4P meeting on 7/9/2015, client was informed to be “open minded” and also there is a possibility she may not be accepted for the housing.

Carla you did not meet expectations regarding documentation from Sept. thru Dec. 2016 and you had a total of 29 overdue cases during this time period. Carla when you do enter your case notes your documentation is organized and understandable in Case Compass. Carla, you have shown the ability to independently connect with collateral contacts and/or external stakeholders during an investigations and maintain working relationship with these contacts; however, your customer service skills could show some improvement as complaints have been received from families, community partners and other outside entities. You have an understanding of when it is appropriate to open a Protective Services case; you consult with your supervisor on a minimum of a monthly basis to address concerns or questions, review your caseload and to discuss significant issues. Carla I would like to see you show improvement in meeting the time frames for face to face and requesting postponements, delays and waiver of face to face visits

Pa is receiving the amount of support necessary to ensure her health and safety needs are met and Pa remains satisfied w/ her services. SC reviewed backup plan w/ Pa, Pa's Cousin Linda, will assist if PAS is not available. This is a new backup plan for Pa. Pa is currently using backup plan until the agency aide starts. SC provided and reviewed the "Who do I contact if. . ." form with Pa. There is no duplication of services. Waiver continues to be the payer of last resort. Pa remains NFCE Waiver appropriate. SC document on SC reviewed Provider Choice, 10 item ISP checklist including rights and responsibilities and right to appeal. SC reviewed Participant Information Packet including: Your rights as a Participant, Your responsibilities as Participant, Participant choice, Applying for HCBS programs, Role of the Service Coordinator, Participant Complaints, How can I find other resources in my Community?, Medicaid (MA) Fraud and Abuse, Who do I contact If…, Abuse, neglect and exploitation, Self -directed services, and Your Appeal and Fair Hearing Rights. The following forms were reviewed and signed: ISP signature page, Provider Choice Form, Voter Registration Opportunity Preference Form and Authorization for Use and Disclosure

On this date, Ms. Lona Williams and Mrs. Belinda Lamb visited Walker Co. DHR, for the purpose of giving information to worker. Ms. Williams stated she was currently staying at her sister, Kathy Tisdale's home. She left her home after her daughter Melissa and her husband were verbal abusive. Her daughter, Belinda Lamb, stated she was taking her mother to the police station to file evection notice. Currently Melissa, and her husband are not at the home and Ms. Williams wants to return home and feels it is a safe environment as long as they do not

Yesterday morning one of Ms. Cortes' neighbors call the staff of Project Home on her. They reported ,she was smoking marijuana in her apartment. Project Home than called DCP&P hotline. Ms. Cortes says, the Social Workers that came out to check the home , did not find anything. As a result, Ms. Anne wants her out of Project Home by the end of the month. Mr. Dotson has given her two options: 1. Go to Eva's Hope Village in Paterson or 2. the YMCA in Newark.
