Worker observed one year old Vincent, he just had a birthday on February 15th. He was observed to be wearing a onsie and was sitting on a high chair. Vincent was eating gold fish crackers. He appeared to be healthy with no visible marks or bruises. Worker was informed that he received four vaccinations and those were his one year old vaccinations. He had his first birthday celebration at his daycare facility. It was a Mickey Mouse theme. Present at the party was his PGM, MGM his father Weylin, mother Katherine as well as the rest of the children in the daycare facility.
Worker spoke to Weylin and Katherine. Worker told them that Katherine is to engage in a psychiatric evaluation and Katherine refuse. She stated she is fine and doesn't need any help from the Division. She said she already has a plan and that they are fine and he case should be close. Delarosa and her daughter Brianna Abreu. Ms. Delarosa explained that she felt that the judge did not read the Division worker's letter. Worker stated that the letter stated that Katherine needed to comply with Division with psychiatric services. Ms. Delarosa stated that court only took a couple of minutes and that the judge said it was an easy case because everyone lives together. She stated that Ms. Reyes and MGM was there and that they stated that Katherine was being disrespectful and scolded her for her behavior.
Ms. Delarosa said her son is being manipulated by Katherine. She stated what Weylin had a nervous breakdown last week and had to go to the hospital. She stated he is attending St. Joes mental health in Paterson NJ. She said her son is not cut out to be a police officer. Worker said that their is a psychological exam that all police officers need to take and if he cannot handle his current life there is no way he can handle the pressures of being an
UCM: CPSW did a home visit again and talked with Annette regarding her relationship with her mother Ms. Clifton. Annette stated that some times that they both disagrees on things and that and than Annette gets upset with her mother. CPSW talked about therapy with Annette and Ms. Clifton. Annette looked at her mother when this therapy talked about family therapy with her mother Ms. Clifton. Ms. Clifton refused to participate with Annette any family therapy. Ms. Clifton stated that Annette has anger issues and that they don't need family therapy at this time. Annette was very quite and mostly was looking her mother's responses regarding family therapy. Valerie called that office several times to inform me of the situation. Ms. Valerie stated that Catina took Bryce to her home on Monday, July 17, 2017, around 4 p.m and did not return until about 10:00 p.m. Ms. Valerie stated that Catina told her that they were going to group at HCAP office for Anger Management. I, Shalina Holmes stated that we do not have a group session on Monday’s at the HCAP office. On 6/9/17 at 1252 Hrs. Officer Villalpando was dispatched to a juvenile problem 1608 10th St. Caller, Nicholas Allen advised that his son, Kaden Boyd (14) who was currently on house arrest, “cut” off his ankle monitor and was last seen getting into a black passenger car. Officer Villalpando arrived and met with Mr. Allen who told him that Kaden had been missing once before while in the Austin area, and that Kaden was involved with the criminal element believing his son is traveling to Austin with an older male, Michael Tabler who had recently been arrested.
This officer has had contact via face to face and phone with the mother, Ms. Evelyn Edwards. Discussions have taken place with her regarding Marquel’s Comprehensive Re-Entry Case Plan. She has participated in a video conference with Marquel since his commitment. She is aware of his placement in the Virginia Beach CPP Program. She has not visited him at the facility, but she has had contact with him via phone. She is currently unemployed. Upon Marquel’s release from DJJ, the anticipated parole plan (CRCP) will be for him to return to her home with intensive supportive services via 294 funding. Intake Worker, FPS White received a phone call from the reporter on 11/14/17. The reporter stated that mom went to jail and got out yesterday. The reporter stated that they are not really concern as to mom going to jail due to it was driving with invalid license. The reporter stated that Jessica Fields' oldest son, Raymond Fields called and told the reporter that the reporter needed to come get the children. The reporter stated that Raymond stated that Jessica was on drugs, pills and meth. Jessica drinks all the time. Jessica got beaten up by the boyfriend, Michael Hays in front of the children. Ms. Alicia appeared less engaged and agitated with allegation. Ms. Alicia stated that she and her daughter are living together and her sister supports her and does child care for her when she is at work. Alicia stated that she works in lab and her work keeps her busy. She stated that she has not spoken to her husband regarding the allegation. However, she stated that she wants John to come back home and they as family resolve the allegation. She appeared to shift her opinion regarding the sexual abuse report about her daughter. CPI asked if she believes her daughter’s opinion and she continued to say “I am following what police wants her to do and has not had a time to discuss about the allegation” She also stated that Laela wants her father at home and she misses him. Ms. Alicia stated that she supports that and wants John to return home. This worker reminded Ms. Alicia that there is still ongoing investigation and that he needs to stay away until she hears from the police. CPI asked if there has been any arrest and she reported no. PER REPORTER: Keshia said she received the following information from a Nurse Practitioner by the name of (Tammie) that works with her. She said according to Tammie, Malia was brought into the clinic today by her babysitter (Linda) to be seen by a physician due to her suspecting that the child has been sexually abused. Tammie said Linda told her that she keeps the child for her uncle due to him being her caretaker at this time due to her mother being in jail. However, Tammie said Linda is suspicious that the child’s uncle may be sexually abuses her. On 09/15/16, there is a Screening Report stating that Ms Rikki Miller (AKA: Bennett) called the GPPD asking to take her child, Keaton to the hospital because he was having bad behavior and she also asked to have her child removed from her home. DHS took that child and placed in the child’s home.
Ms. Pollard wanted to know when Javon'Tay would receive the DD waiver. ISC explained the process, than she asked about receiving a case manager. She said she was told by the other lady he would receive a case manager, which would take him out to his appointments and take him places.
The worker contacted Brittany Coleman, the sister of Jessica Farrand. Mrs. Coleman stated” I don’t think the children are being properly taken care of because Jessica is erratic, emotionally unstable, and she and Steven Burke both have a drug and alcohol problems”. Mrs. Coleman stated “Jessica is in an abusive relationship with Steven and he has put a gash on her forehead so bad my mother had to take her to the hospital. RCG Rose Campos reports no health problems. Ms. Campos also reported that she has had no problems with Elijah at home. However, I am concerned that Elija is not safe in his present home with, the maternal grandmother due to Austin’s behavior. Ms. Campos reports no physical harm to Elijah by Austin, but Elijah has witnessed Austin’s violent and abusive behavior to Ms. Campos. I am concerned about Austin’s exposure to this violent and abusive behavior to his grandmother.
Morris told me that she hasn’t been diagnosed with any type of psychological disorders. Morris has never attempted to commit suicide in the past. Based on Morris’ statements made to me, I formed the opinion that she was a danger to herself. I placed Morris on a WIC 5585-Mental Health Hold. I informed Social Worker Lauren of the circumstances.
SSA received a phone call from Verna Goecke-PRPV inquiring if the Bond family was aware of Katlyn’s discharge from the facility. She stated that she had not heard from Mr. Bond regarding the discharge notice and according to Katlyn she had not discussed the discharge arrangements with her parents. SSA shared that two messages had been left on Karen Kohn’s cell phone. We discussed that in the event the family did not arrive to the meeting location this morning, PRPV was willing to transport Katlyn to the family home. Verna asked SSA to email the family’s home address in the even they needed to transport Katlyn to the home.
