Analyzing a Family Health Assessment A family health assessment is a process by which a nurse evaluates and describes the health status of a given family. It is a framework that helps to identify areas of potential risk for illness, opportunities for health education and actions needed to address these (World Health Organization, 2001). Specifics covered in a nurse led family assessment will include family history, perceptions about health, reports, health records, and any clinic test results. Father gets up once during night to empty his bladder and mother reports problems with bladder leakage from stress incontinence(Johnson family, personal communication, June 24, 2012).
Activity-Exercise Pattern: Father, son and daughter Johnson tolerate physical activity very well and all have shared a lifelong interest in swimming and biking. All would agree that regular exercise reduces stress and improves coping abilities for them. Mother however has never participated in regular physical fitness and does not tolerate strenuous activities very well. Strenuous activities are avoided now primarily because of the (bladder) stress incontinence problem she has. She denies feeling any benefits of stress reduction when she is active (Johnson family, personal communication, June 24, 2012).
Sleep-Rest Pattern: The Johnson family reports getting 7-9 hours of restful sleep each night. Mother works a swing shift and has more difficulty feeling rested after getting out of bed. She reports having an occasional glass of wine to fall asleep. No one else in the family uses sleep aids or alcohol to fall asleep (Johnson family, personal communication, June 24, 2012).
Cognitive-Perceptual Patterns: The Johnson women have established relationships with primary care providers and arrange most of their care needs through their PCPs. The men however seek treatment only when unexpected health problems arise.

It focuses on individual behaviour towards a wide range of social and interventions. It also shows that health promotion is aimed at individuals whose health is at risk from any diseases from lifestyle factors, which lets individuals take control over their health. Honesty is something I value in another person, and I have struggled with service users who haven’t been honest about something. I have tried hard to understand the reasons why someone would not be honest with me. Usually after I have spoken to another member of staff, and heard their input and theories I can begin to see why and move on from the incident.

Assessments are instrumental in determining health patterns. Edelman, Kudzma, & Mandle (2014) states that the nursing assessment determines the health status of individuals. Health assessments require a holistic approach to ensure that each area of the patient’s daily living is covered. Each of Gordon’s functional health patterns target the emotional, physical, and spiritual aspects of each person. Assessments will differ based on each person’s self-perception and individual needs. When developing individual plan of cares, and assessing Gordon’s health patterns it is mandatory that the patient’s values and beliefs are accounted for. Lack of consideration obstructs rapport and impedes future communication efforts initiated by the healthcare provider.
Assessments of family’s health are to help focus on the control and prevention of diseases. Health assessment assist in determining an individual’s perception of their quality of life along with the family’s perception as a whole. This also helps to define a person’s level of fulfillment when looking at his or her health status. One way that we as nurses can gather information on family and their health is by using Friedman’s family assessment model. This model includes patterns such as values, perception of health, nutrition, sleep/rest, elimination, activity/exercise, cognitive and sensory perception, self-perception, role relationship, sexuality, and coping abilities. This tool helps the nurse outline, in an organized manner, of information gathered on a family’s health.
